We wish to make this process as easy as possible for you. This is how to refer a patient to a dermatologist in 3 easy steps:

Step 1: COMPLETE AND FAX DERMATOLOGY REFERRAL FORM 

This form is short and only focuses on the relevant medical history we require. It should only be completed by the referring veterinarian or an animal health technician. If completed fully, it is not necessary to attach a copy of all medical records.

Please fax the completed form + additional information to 250-590-8415.

Please advise your clients that Eagle Rise Animal Hospital will contact them directly to set up the appointment.

Step 2: PREPARE YOUR CLIENTS FOR THEIR VISIT

Your clients are invited to complete a thorough history questionnaire in advance of their arrival at Eagle Rise Animal Hospital and bring this completed form to the appointment.

IMPORTANT NOTES

If the pet is referred for intradermal allergy testing, please note that certain medications need to be discontinued prior to the appointment.

If drug withdrawal is not possible, a dermatology consultation is still a valuable option.

Step 3:  REVIEW REFERRAL REPORT

A referral report will be sent to your office within days of your patient’s appointment. If there are any questions or concerns after you have read the referral report, please contact Eagle Rise Animal Hospital and an Animal Health Technologist will be available to answer any questions or seek advice from the veterinary dermatologists.
